  • Your SUD treatment records generally may not be disclosed or used in any civil, criminal, administrative, or legislative proceeding against you without your specific written consent or a qualifying court order.

  • Federal law and regulations protect the confidentiality of substance use disorder patient records at least as strictly as, and in some respects more strictly than, general medical information.

  • We will not release information that could identify you as a person who has received SUD-related services without your specific written consent, except in narrow circumstances permitted by law (such as a medical emergency or as required by law).

You have the right to:

  • Request a restriction on certain uses or disclosures of your PHI (we are not required to agree, except in certain circumstances involving payment restrictions for services paid out-of-pocket in full).
